Hazel Dale Lewis Faulkner, 86, of Maysville, went home to be with the Lord, Thursday afternoon, August 18, 2016, at Maysville Nursing and Rehabilitation Center.

She was born November 30, 1929, in Beckley, West Virginia, a daughter of the late Elmer Dale and Nannie Belle Ratliff Lewis.

Hazel was a member of Vanceburg Christian Church and the Vanceburg Historical Society. She enjoyed traveling and reading.

In addition to her parents, she was preceded in death by her husband of 45 years, Orah Faulkner; one brother, Elmer “Buddy” Lewis; and one sister, Helen Ruark.

Hazel is survived by three sisters, Lorene Free of Hebron, Carla Lewis of Florence, and Dorothy Hunter of Vanceburg; two special nieces who cared for her during her sickness, Patsy Lewis and Carolyn Kinney; and several nieces and nephews. She also leaves many other family members and friends who will sadly miss her.

Services will be at 2:00 p.m. Sunday, August 21, 2016, at Dickerson Funeral Home in Vanceburg with Bro. Kevin Cornette officiating. Burial will follow in Lewis County Memory Gardens at Black Oak.

Friends may visit from Noon until 2:00 p.m. Sunday, August 21, 2016, at Dickerson Funeral Home, 110 West Second Street in Vanceburg.

Family and friends will serve as pallbearers.
